Services
Payment

After each interview/session at CNLD, you will receive an itemized statement of services. Please plan to pay at the time of service. If you need to make alternative payment arrangements or need to work out a payment plan, please discuss this with the clinic director at the initial interview; therapy clients should discuss this with their therapist.

Insurance Reimbursement

CNLD does not bill insurance companies directly. It is the client’s responsibility to check withhis/her insurance company prior to the appointments to determine coverage and reimbursement.

Payment is expected at the time of service. We can providecodes to you in advance of your initial meeting so you can find out what portionof our services may be covered in your policy. We will provide you with an itemizedstatement of services after each appointment that will include appropriate procedure, diagnosis, and billing codes for your insurance company. Once you have paid the CNLDinvoice, you will receive a copy of it, marked "paid." You may then submit this invoice to your insurance company for reimbursement.

Cancellation Policy

If you need to cancel an appointment, please provide at least 48 hours notice so that we can reschedule the appointment time. If an appointment is canceled with less than 48 hours notice, you will be charged for a portion of the time (excluding emergencies).

If you miss an appointment, you will also be charged for a portion of the missed time. Therapy clients will be charged for the missed session if less than 24 hours notice is given.

Coming to see you for an evaluation was so helpful and I’m so happy that I did this. After struggling for years with ADHD – but not knowing that’s what it was – and almost completely ruining our marriage because of it, your diagnosis helped more than you could know. Now I know that it’s not just me – the diagnosis has turned our lives around and helped me feel more accomplished at work. Thanks again for everything.
